Pre-Travel Information Form
All travel vaccinations need to be requested 10 weeks in advance before you travel.
- You need to complete a Travel Form (below) and give it to Reception in person.
- Reception gives the completed form to Practice Nurse. They will look at your vaccination history.
- If you are due any vaccinations, the Practice Nurse will instruct the receptionist to contact you to book you into the Travel Vaccination Clinic.

Vaccine Costs
- Meningitis Vaccine - (£15.00 for the certificate)
- Hepatitis Vaccine (course of 3) = £40.00 each
To receive your Vaccination Certificate, make sure you bring cash with you when attending the Travel Vaccination Clinic!
Cancelling your Travel Vaccination Appointment
If you cancel your travel vaccination appointment, you will not be able to re-arrange it, therefore you will have to contact a Private Health Clinic. Please note you will be charged at a Private Health Clinic.
